Wellness Plan Fees
$155.00/month: Receive up to 4 visits per month.
Must buy 3 months in advance.
Pay $155.00 Monthly after the first 3 months with continued care and no more than a 2 month absence of treatment.
Additional visit/s: Beyond 4 in the month $30.00 each.
